Case Management is a service that coordinates community services to help frail older adults stay safely in their own homes. Case Management will not take the place of family, friends or neighbors who are already helping. It will support their efforts and fill in where needed.
In order to understand an individuals needs, a visit will be arranged in their home. They will be asked questions to assess their situation and determine which services meet their needs. They choose the services they want and their Case Manager will help arrange and coordinate the services. The Case Manager will continue to be involved, checking in with them to assure that they continue to receive the assistance they need.
