Program for Assertive Community Treatment (PACT)

PACT is a team of professionally trained individuals working to provide comprehensive home and community based mental health and vocational service to persons with chronic and persistent mental illness. Services are individualized to meet the unique needs of each consumer and address all areas related to successful community living. The PACT team provides both case management services and direct, hands-on support services.

PACT Goals
•Lessen or eliminate the debilitating symptoms of mental illness and minimize or prevent recurrent acute episodes of the illness
•Facilitate successful community living by providing intensive community supports as needed
•Assist clients in meeting basic needs and enhance quality of life
•Broaden the support system available to the consumer, including professionals, family members, and significant others
•Assist with and facilitate vocational and social roles in the community
