Supported Community Living (SCL)

Through the assessment and goal planning process, Supported Community Living helps clients identify their strengths, areas that they need to work on and priorities that they have. SCL staff and an interdisciplinary team (client driven) develop goals and action steps and identify which provider will be assisting the client in meeting these goals. Typically, the SCL staff would be responsible in helping them in the direct service needs. Outcome measures indicate that it has been successful as the past two consumer surveys came back 100% satisfied. The goal of SCL is to provide the tools and develop skills that enable people with mental health disabilities to live, learn, work, and socialize in the community. The intent of SCL is to maximize clients’ ability to live independently in the community while minimizing service needs and increasing natural supports.
