Case Management is a service that coordinates community services to help frail older adults stay safely in their own homes. Case Management will not take the place of family, friends or neighbors who are already helping. It will support their efforts and fill in where needed.

In order to understand an individuals needs, a visit will be arranged in their home. They will be asked questions to assess their situation and determine which services meet their needs. They choose the services they want and their Case Manager will help arrange and coordinate the services. The Case Manager will continue to be involved, checking in with them to assure that they continue to receive the assistance they need.

The Elder Abuse Prevention and Awareness (EAPA) program focuses on the prevention, intervention, detection and reporting of elder abuse, neglect and financial exploitation of individuals 60 years of age or older. Anyone concerned about a vulnerable adult may also contact EAPA staff.

Services:

  • Respond to reported concerns of older adults who are at risk of, or experiencing, abuse, neglect or financial exploitation
  • Network and coordinate community resources to respond to the needs of this population
  • Collaborate and be a resource for all concerned individuals, case managers, physicians, law enforcement, county attorneys, DHS, domestic violence agencies and community providers
  • Increase public awareness on elder abuse issues.

The Family Caregiver Support program provides information regarding appropriate services for themselves as well as the person receiving care, education related to specific issues which can empower family caregivers, and counseling to support the common emotional challenges that face many caregivers. Limited funding for respite and other items that support the caregiver.

Services:

  • Assessments provided over phone, at office, or in home as necessary
  • Information about available services and assistance in establishing desired services
  • Emotional support
  • Education on diseases and memory loss to increase the caregiver’s confidence
  • Respite funding on a limited basis
  • Limited funding for equipment, rent, utilities to support the caregiver

Eligibility:

  • No age restrictions for caregivers of an individual 60 years of age or older
  • Age restrictions for other caregivers:
    • Adult 55 years of age or older and caregiver of an individual at least 18 years of age with a disability
    • Adult 55 years of age or older and caregiver of a minor child family member under 18 years of age

Intake Process:

Caregivers can call at any time. An application is required only when seeking consideration for emergency funding.

The Older Worker Employment Program in partnership with Iowa Vocational Rehab focuses on assisting adults, 55 years of age or older, that have any type of disability to obtain employment.

Services:

  • Employment goal setting
  • Assistance in building a resume and identifying references
  • Aid in locating opportunities and applying for open positions
  • Arrange informal interviews and job shadowing opportunities
  • Interview preparation

Eligibility:

Individuals must be at least 55 years or older and diagnosed with a disability. Participants must be working with Iowa Vocational Rehabilitation Services (IVRS).
