Medical providers make a referral to 1st Five and a developmental support specialist contacts the family to discuss the family’s concerns. 1st Five developmental support specialists provide resources and referrals to help meet the needs of the family.

Medical providers make a referral to 1st Five and a developmental support specialist contacts the family to discuss the family’s concerns. 1st Five developmental support specialists provide resources and referrals to help meet the needs of the family.

Services:

  • Supporting the population of children birth to age five (up to a child’s fifth birthday).
  • Increasing use of surveillance and standardized developmental screening by partnering with (engaging) primary care providers.
  • Providing a one-step referral resource for primary care providers.
  • Connecting referred children (and their parents/caregivers) to existing services in their local communities.
  • Keeping primary care providers informed about children’s progress.
  • Supporting healthy social, emotional and cognitive development of children birth to 5.

Eligibility:

1st Five is your partner in supporting healthy cognitive, social, and emotional development in Iowa children ages 0-5. American Home Finding Association’s 1st Five program accepts referrals from medical clinics in Appanoose, Davis, Jefferson, Keokuk, Mahaska, Marion, Monroe, Van Buren, and Wapello counties.

  • Developmental screens
  • Mental health or behavioral therapy referrals
  • Child development information/education
  • Community resource referrals/information
  • Financial resources
  • Connection to resources to meet individualized family needs or concerns

Eligibility:

Families with children ages birth to 5.

Intake Process:

Referrals are made through primary care providers. Please contact your child’s primary care provider if you are interested in 1st Five.
