Eligibility:
Families enrolled in Perry CSD may apply. If recipient of LIHEAP, WIC, or Head Start automatically qualify but must complete application regardless. Children ages 0-14 qualify.
Intake Process:
Complete online form or call to schedule appointment during M-F 9am-3pm. Application will open mid-October and remain open through mid-Nov.
