Siouxland Community Health Center (SCHC) is a nonprofit organization dedicated to meeting the health care needs of the community and supports the philosophy of the patient-centered medical home. SCHC is accredited through the Accreditation Association for Ambulatory Health Care (AAAHC). To earn accreditation, SCHC participates in an ongoing self-evaluation, peer review and education to continuously improve its care and services.
